Our Approach to Providing Support and Resources
Family & Children Services pairs youth and young adults with behavioral health needs or developmental disabilities with a supportive point of contact for care coordination and resource navigation.
Program Overview
Our case management efforts focus on working with minor youth and their families as well as young adults (ages 18-26 years of age) who have behavioral health needs or developmental disabilities.
This approach helps individuals and families successfully participate in a community environment by providing services that are responsive, respectful, and flexible.
We empower individuals and families with strength-based strategies and the incorporation of multiple services needs to enable them to achieve their goals, direct their own lives, and reduce dependency on service providers. Our case management philosophy is responsive, respectful, and adaptive to individual needs and preferences.
Family & Children Services’ case management services include:
- Crisis intervention and support.
- In-home and community-based treatment services.
- Clinical assessment and determination of youth and family needs.
- Assistance in developing individualized, family-centered plans
- Co-occurring treatment.
- Parenting/family education, information, and support.
- Advocacy, referral, and support within community and educational systems.
- Coordination of psychiatric services.
- Individual and family support services focused on community integration.
- Support in developing and maintaining independence.
A Care Coordination Approach to Case Coordination
Case managers coordinate and monitor services and supports within community, educational, legal, and employment systems by:
- Using a strength-based, needs-driven approach.
- Encouraging client choice throughout service involvement.
- Helping clients identify and build relationships with natural supports.
- Providing advocacy, community-based referrals, and support.
- Partnering with clients to create and sustain community involvement.
- Promoting wellness, recovery, and self-determination.
- Recognizing the client’s personal stage of change and addressing co-occurring (mental health and substance use) issues.
Eligibility and Referrals
Case management services are available to minor youth and their families as well as young adults (ages 18-26 years of age) who have behavioral health needs or developmental disabilities.
Case Management services are provided through a contract with INTEGRATED SERVICES OF KALAMAZOO (ISK).
