Caring for Individuals and Families as a Whole
The Family And Community Treatment (FACT) through Family & Children Services serves youth, as well as their families, in Kalamazoo County experiencing difficulties in multiple areas of life due to behavioral health needs.
Program Overview
FACT is an intensive, home-based treatment service that works with the entire family of a youth with a mental health diagnosis. By providing the resources and skills of a multi-disciplinary team, FACT builds on youth and family strengths to increase personal and community support and promote healthy family functioning.
The program is flexible and based on specific needs and concerns.
FACT
FACT provides case management services for the entire family, offering flexible scheduling and a variety of treatment services including:
- Voluntary, home and community-based treatment services.
- Clinical assessment and determination of individual and family needs.
- Assistance in developing individualized, youth and family-centered plans.
- Integrated mental health treatment.
- Co-occurring treatment for substance use.
- Parenting education, information, and support.
- Advocacy, referral, and support within community and educational systems.
- Coordination of psychiatric services.
- A variety of clinical interventions including evidence-based models.
- Family support services focused on family functioning and community integration.
A Care Coordination Approach to Case Coordination
Under the supervision of a FACT Supervisor, the team works with the entire family to coordinate and monitor services and supports within community, educational, legal, and employment systems by:
- Using a strength-based, needs-driven approach.
- Encouraging client choice throughout service involvement.
- Helping clients identify and build relationships with natural supports.
- Providing advocacy, community-based referrals, and support.
- Partnering with clients to create and sustain community involvement.
- Promoting wellness, recovery, and self-determination.
- Recognizing the client’s personal stage of change and addressing co-occurring (mental health and substance use) issues.
Eligibility and Referrals
The FACT program is open to children and families living in Kalamazoo County.
Services can be requested through Integrated Services of Kalamazoo (ISK) by families in Kalamazoo County. A FACT Supervisor will guide families and service providers through the steps to make a referral and discuss whether FACT services are a good match for a child and family.
Funding
FACT services are provided through a contract with Integrated Services of Kalamazoo.
